Am I eligibile for care?
To be Eligible for care at Crossroads Center you must be:
◦A resident of Okaloosa or Walton County
◦A patient without health insurance
◦Household income does not exceed 200% of Federal Poverty Guidelines
◦A U.S. Citizen or legal resident
◦An Adult (18 -- 64 years of age
Documentation required:
◦Photo ID
◦Proof of Okaloosa or Walton County residency (utility stub, mailing with address, etc.)
◦Income stubs for the previous four (4) weeks
After financially qualifying, you will be given an appointment.
Crossroads is not a walk-in/immediate care clinic.
Because we seek to provide ongoing, quality care for our patients, each patient will be seen by appointment only.

Volunteer Status: The clinic will not bill for any services rendered. Workers, Clinical Personnel, Administrative staff and Board of Directors are volunteers of Crossroads Center.
