More About Crossroads Center
Clinic Operations: Crossroads Center Inc. opened January 20,2007. Crossroads is not a walk-in/immediate care clinic. Patients are seen by appointment only.
Financial screening: is required before an appointment is made. Call 850-389-3015 for the next scheduled Financial Screening date or for more information.
Documentation required:
- Photo ID
- Proof of Okaloosa or Walton County residency (utility stub, mailing with address, etc.)
- Income stubs for the previous four (4) weeks
Volunteer Status: The clinic will not bill for any services rendered. Workers, Clinical Personnel, Administrative staff and Board of Directors are volunteers of Crossroads Center.
