Crossroads Center, Inc. is a free clinic for low income, uninsured adult residents in Okaloosa County. Presently there is no place for our residents to go to get care except through the emergency rooms in our hospitals. Many residents wait until their condition is critical, requiring advanced and expensive treatment.
Crossroads Center is a community based effort. We are developing partnerships with the State of Florida Department of Health, Okaloosa County Health Department, Fort Walton Beach Medical Center, Twin Cities Hospital, Okaloosa Walton College, churches, businesses, civic and governmental agencies of our areas.
The clinic provides the following services:
To be eligible to participate with Crossroads Center, you must:
Be a resident of Okaloosa County, Florida;
Be a patient without health insurance;
Have household income not exceeding 200% of Florida State Poverty Guidelines;
Be a U.S. citizen or a legal resident; and
Be 18 years of age or older.
Crossroads Center Medical Clinic will opened in January 20, 2007. Limited administrative staff will be available Tuesday through Friday, 8 a.m. to 4 p.m. We are currently closed Saturday through Monday.
